Experimentierkonsum und problematischer Drogenkonsum

Fragt man Menschen in Österreich, ob sie irgendwann in ihrem Leben schon einmal illegale Drogen konsumiert haben, dann werden Konsumerfahrungen mit Cannabis am häufigsten genannt. Schätzungsweise 33 bis 50 Prozent geben an zumindest einmal im Leben Cannabis konsumiert zu haben. Außerdem haben etwa drei bis vier Prozent der Österreicher*innen schon einmal in ihrem Leben Ecstasy, Kokain, Amphetamine, biogene Drogen oder Schnüffelstoffe probiert. Ein bis zwei Prozent haben schon einmal Methamphetamin, LSD oder neue psychoaktive Substanzen (NPS) konsumiert. Tendenziell geben Männer bei fast allen Drogen öfter an, sie bereits probiert zu haben als Frauen. Bei einem Großteil dieser Personen war der Konsum aber lediglich eine vorübergehende Probiererfahrung.

Generell beschränkt sich der Konsum illegaler Substanzen aber meist auf eine kurze Lebensphase. Klar abzugrenzen von Probierkonsum ist der problematische Drogenkonsum. Darunter wird ein wiederholter Drogenkonsum verstanden, der Schaden (Abhängigkeit, aber auch andere gesundheitliche, psychologische oder soziale Probleme) für die Person verursacht oder sie einem hohen Risiko, einen solchen Schaden zu erleiden, aussetzt.

Der Gebrauch mehr als einer Droge mit Beteiligung von Opiaten (z. B. Heroin, Substitutionsmittel) spielt beim problematischen Drogenkonsum in Österreich die zentrale Rolle. Aktuell konsumieren zwischen 31.000 und 37.000 Personen problematisch Opioide (meist in Kombination mit anderen illegalen Drogen, Alkohol oder Psychopharmaka). In etwa drei Viertel der Betroffenen sind Männer. Die Daten deuten darauf hin, dass weniger junge Menschen in einen problematischen Opioidkonsum einsteigen als in früheren Jahren.

Es gibt Hinweise auf einen regional gestiegenen Kokainkonsum in Österreich. Die Konsummuster für Ecstasy-Tabletten scheinen sich nicht geändert zu haben, aber aufgrund der (oft unerwartet) hoch dosierten Ecstasy-Tabletten ist er risikoreicher geworden und hat zu einer Reihe von Todesfällen geführt.

Cannabis

Cannabis ist weltweit, in allen EU-Ländern und in Österreich die mit Abstand am meisten konsumierte illegale Substanz.

Schätzungsweise hat ein Drittel bis die Hälfte der Österreicher*innen bereits einmal in ihrem Leben Cannabis konsumiert. Nur 5% der Österreicher*innen geben an, in den letzten zwölf Monaten Cannabis konsumiert zu haben; 3% in den letzten 30 Tagen. Wobei der Cannabiskonsum bei Personen im Alter von Anfang bis Mitte 20 am weitesten verbreitet ist: Um die 15% in diesem Alter berichten von Cannabiskonsum innerhalb des letzten Jahres. Gegen Mitte bis spätestens gegen Ende der dritten Lebensdekade nehmen aktuelle Konsumerfahrungen bei Frauen deutlich ab, bei Männern ist das erst mit Ende der vierten Lebensdekade der Fall. Männer berichten generell etwas häufiger von Cannabiskonsumerfahrungen als Frauen.

Personen mit höheren Bildungsabschlüssen sowie Personen aus mittleren Städten (mindestens 50.000 Einwohner*innen) und Großstädten (mindestens 250.000 Einwohner*innen) berichten deutlich häufiger von Erfahrungen mit Cannabis als solche mit mittleren bzw. niedrigen Bildungsabschlüssen und Personen aus ländlichen Regionen

Unter den 15- bis 17-jährigen Schüler*innen haben in Österreich 20% schon einmal in ihrem Leben Cannabis probiert. Beim aktuellen Cannabiskonsum, dies bedeutet während der letzten 30 Tage verringert sich der Prozentsatz auf 11%. Die Mehrheit jener, die Cannabis konsumiert haben, ist dabei in einem Bereich anzusiedeln, der als Probierkonsum zu klassifizieren ist. Der erste Kontakt mit Cannabis findet in der Regel deutlich später statt als der erste Kontakt mit legalen Substanzen wie Alkohol oder Nikotin. 2% der 15- bis 17-jährigen Schüler*innen weisen jedoch einen riskanten Cannabiskonsum auf.

In den letzten Jahren hat sich das Bewusstsein für die Auswirkungen des langfristigen, hochfrequenten Konsums von Cannabis geändert und die Zahl der ambulanten Behandlungsnachfragen wegen Cannabisproblemen ist seit 2010 gestiegen. Was auch mit einer Entstigmatisierung des Cannabiskonsums erklärt wird.

Die Gesundheitsrisiken von regelmäßigem Cannabiskonsum werden heute differenzierter bewertet. Der Konsum kann bei psychischer Instabilität, exzessivem Gebrauch oder zu frühem Einstiegsalter durchaus zu Abhängigkeit führen, die mentale Gesundheit beeinträchtigen und sogar Psychosen auslösen.
