E-Zigarette zur Entwöhnung verlängert Abhängigkeit

Welchen Effekt hat der Umstieg von Tabakprodukten auf E-Zigarette auf die Nikotinabhängigkeit? Verlängert sie die Sucht eher oder fördert sie den Ausstieg? Diese Frage nahm ein Review unter die Lupe.

Abhängigkeit von Nikotin bleibt eher

Die Autor*innen verglichen die vier verfügbaren hochwertigen Studien, die E-Zigaretten mit pharmazeutischer Nikotinersatztherapie verglichen. Insgesamt waren darin 1.598 Proband*innen im Alter von 41 bis 54 Jahren eingeschlossen. Sie rauchten vor dem Aufhörversuch zwischen 11 und 21 Zigaretten täglich.

Fast neun Mal so häufig abhängig geblieben

Untersucht wurde, wer nach dem Loskommen von Tabakzigaretten noch abhängig von Nikotin war und mit welcher Methode die Entwöhnung besser gelang. Wer statt der konventionellen Zigarette auf E-Zigarette umstieg, hatte zwar eine geringfügig höhere Wahrscheinlichkeit, nicht mehr konventionelle Zigarette zu rauchen. Konkret war die Wahrscheinlichkeit 1,58-fach erhöht im Vergleich zu Nikotinersatztherapie.

Im Gegenzug scheint die E-Zigarette aber die Entwöhnung von Nikotin zu erschweren und eher zu einer verlängerten oder dauerhaften Sucht zu führen. Die E-Zigarette als Hilfe bei der Tabakentwöhnung führte zu einer 8,94-fach erhöhten Wahrscheinlichkeit für weitere Nikotinabhängigkeit. Im Vergleich zu pharmazeutischer Nikotinersatztherapie bleiben mit der E-Zigarette demnach fast neun Mal so viele Menschen abhängig von Nikotin.

Die E-Zigarette als therapeutische Intervention einzusetzen, kann also den negativen Effekt einer dauerhaften Suchterkrankung haben, so der Review.
